Looks like an early form of PTO, power take-off. Is that an additional chain going up to the hose reel? I can see that being used to rell the hose back in. Just lift the bike on rear-wheel stand, flip chain on, and start pedaling.
I wonder if you can run hose with it. Looks like you might be able to hook up one end, than push the bike to lay out the rest.
I ran hose one summer afternoon in my racing days after I spotted a fire on my dad's property, deep in the woods. It was when there have been no rain for weeks. The fire crews had been firefighting non-stop. They loved having this fit young man willing to run the hoses. And I quickly found out it was hard work!
It turned out I ran all that hose for nothing. The wind shifted and the fire veered away from us towards a very rocky area and we started losing the battle. (The truck could have come in from the front of the fire from another street. I had them com in from our property because 1) there was goo access from the back and they could get the truck fairly close and 2) a pond was nearby to refill the truck from. This was a tanker. We were a very long ways from the nearest hydrant.)
Just as things started looking really bleak, it started to rain. Rain never felt so good. We all just stood there and said our thanks. The next day, a jeep left our woods filled completely with fire hose that I'd run! Later the entire parking lot at the fire station was covered with hoses laid out neatly to dry. (Far more than I ran. Many jeep loads.)

I downloaded that pic and blew it up. That bike is impressive. That hose rack is joined to the bike tubes with lugs. The seatstay attachment looks 1980s Italian. The siren is on a brazed mount (and may itself be brazed on).
Put those wheels on a SS CX bike 10 years ago and no one would notice if it were not for the red. (Of course, the hubs are steel. I'm thinking aluminum was still a few decades away when this was built. 1910 perhaps?)
